DISCOUNT DRUG CARD GROUP INSURANCE BENEFIT

Members enrolled in the SNAME Discount Drug Card Program receive a 16% discount off their prescription drug cost because of their participation in this program. Any member wishing to enroll for this benefit should contact eromanelli@sname.org specifying the address where the brochure/enrollment card should be mailed.

HIGH-LIMIT ACCIDENT PLAN

Effective July 1, 2001 the insurance carrier for the SNAME High-Limit Accident insurance plan became New York Life Insurance Company. Because of unfavorable claims experienced during the past year, particularly among older insured, US Life (the previous carrier) requested certain benefit and rate changes that we felt were not warranted. As a result, other carriers were asked to bid on the Plan.

Under the new Plan all current benefits will remain as is. In addition, New York Life will be adding and/or enhancing the following benefits at no additional charges to the member.

The Rehabilitation Benefit will be enhanced to cover insured spouse and children.

The Repatriation Benefit will be expanded to cover expenses incurred to return the insured person's body to his/her state of residence. The maximum benefit has been increased from $5,000 to $25,000.

The Educational Benefit will be expanded to cover educational expenses incurred by the insured spouse in addition to any child.

A new Adaptive Home/Auto Benefit will be included that will provide for the reimbursement of expenses necessary to modify an insured's home and/or car to accommodate physical disabilities resulting from a covered accident.

Any member wishing to participate in this program should contact Seabury & Smith, toll-free at 1-800-424-9883 or email sname@seabury.com.
