Employment Information
Please provide a detailed account of each position. This should include starting and ending dates, position title, name of firm, description of duties, total time in position.


References
Be sure the proper number of references are provided and that these individuals meet the requirements as stated on the form for each grade of membership.

Those persons unable to provide the required references and signature shall sumbit the names of three non member references and state the reason for this deviation. The Applications Committees shall consider this situation and act in their best judgement.


Application Review

All applications for new members are reviewed by our Applications Committee which meets on a bimonthly basis. You will be notified shortly after review. If accepted, you will receive a package of pertinent information including a membership card, membership certificate and other member benefit information including, The User's Guide to SNAME Benefits and Services.


Requesting Change of Grade
Check the appropriate box on the application form and complete all portions of the form, except as indicated in sections identified for Education History and Employment Information. In these two sections you need only provide information which is NEW since you prepared your origina; application.

New endorsements are required for a change of grade. Be sure this portion is completed by the proper number of individuals and that these persons meet the requirements for grade as listed on the form.

All requests for change of membership grade are reviewed by the Application Committee which meets on a bimonthly basis. You will be notifed shortly after the review. If approved, you will receive a new membership card showing your new grade. A new membership certfificate will follow.


Reinstatement
A MEMBER who has resigned or whose membership has ceased for non-payment of dues may be reinstated by the Applications Committee by any of the following:

    1. By paying back dues from the year of severance of membership with reinstatement to take effect as of the year of original approval; or

    2. By paying a one-time administrative fee of $50.00 with reinstatement to take effect as of the year of original apporval minus the amount of years of broken membership. Members shall have the right of exercising this option one time during the life of his or her membership; or

    3. By making a new application for membership with reinstatement to take effect as of the year of re-approval.

In any event, use the following application in abbreviated form as appropriate. If membership has lapsed for two or more years, it will be necessary to complete the two sections identified as Education Histry and Employment Information.
